Kids Summer Camp

Please acknowledge our safety waiver and sign below:

I/We hereby understand and acknowledge that the training, programs, and events held by Central Toronto Wrestling Club may expose me/my children to risks, including accidents, injury, or illness.
I/We assume all risks of injuries associated with participation including, but not limited to, falls, contact with other participants, bodily injuries, and all other such risks being known or not known to Me/Us.

I/We hereby acknowledge my responsibility in communicating any physical and psychological concerns that might conflict with participation in provided activities. I/We acknowledge that I am physically fit and mentally capable of performing the physical activity I choose to participate in.
After having read this waiver and knowing these facts, and in consideration of the acceptance of my participation with Central Toronto Wrestling Club services to me, I agree, for myself and anyone entitled to act on my behalf, to hold harmless, waive, and release Central Toronto Wrestling Club its officers, agents, employees, organizers, representatives, partners, owners, instructors, and successors from any responsibility, liabilities, demands, or claims of any kind arising out of my participation in Central Toronto Wrestling Club training, programs, and/or events. I/We acknowledge the risks of a personal contact course during COVID-19 and assume all related risks associated with taking this course. Participants agree to hold harmless Central Toronto Wrestling Club and its directors of any illness or sickness related to COVID-19. By my signature, I/we indicate that I/we have read and understand this Waiver of Liability. I am aware that this is a waiver and a release of liability and I voluntarily agree to its terms.

I/We agree that any pictures, audio, or visual recordings taken of me or my children in connection with Central Toronto Wrestling Club can be used for publication, promotion, articles, and advertisement without additional consent and without compensation at this time or any other time. I have read and understand this release and agreement and agree to its provisions. I am not under the influence of any drugs, alcohol, or other intoxicants. I am not suffering from any illness or incapacity.

I am over 18 years of age. (If not over 18 years of age, parent or guardian must sign.

Statement of Consent & Waiver 

I hereby agree and declare that I am the legal parent/guardian of the above-named child and hereby consent to the child's participation in the activities that are described to me in the registration process. I understand that actions of the kind described may result in physical injury to my child but nonetheless specifically request that he or she be allowed to participate.

If the above-named child requires any emergency medical treatment or procedures during the activities, I hereby consent to the activity supervisor(s) to make any decision and take any action to arrange for such procedures or treatments at the discretion of the activity supervisor(s).

I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer camp activities and I consent to the participation of the above-named child in these activities.

Any injury that may occur to the child without the fault of the camp management and activity supervisors, the camp management cannot be held responsible for any harm that may occur to the child without the fault of the camp management and activity supervisors.
